Objective — observe

  • Behavioral indicators of pain — guarding, grimacing, restlessness, or splinting; for nonverbal patients, the PAINAD components.
  • Sedation level and respiratory status on an opioid regimen.
  • Functional observation — gait, transfers, and positioning limited by pain.
  • The site — swelling, deformity, wound, or skin change relevant to the pain.
