10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

  • Reassessment score after an intervention — for example pain ___/10 → ___/10 after the analgesic, with the functional effect.
  • Tolerance and any adverse effects of the administered or coordinated medication.
  • Teaching effectiveness: teach-back percentage or return demonstration of the regimen and safety plan.
  • Functional response — improved ambulation, transfers, or sleep.
  • The plan for reassessment timing and what will be re-evaluated next visit.
