Subjective — ask

  • Symptoms associated with an abnormal value — chest pain, palpitations, dyspnea, dizziness, headache, visual change, chills, or weakness.
  • Recent activity, intake, caffeine, nicotine, or missed or extra medication doses that may affect a reading.
  • The patient's usual home values and any home-log readings.
  • Adherence to and adverse effects of medications that affect vital signs (antihypertensives, rate-control agents, diuretics, antipyretics).
  • Functional tolerance and any change from baseline.
