Abnormal findings and what they may signal — each warrants Mode B focused documentation and, where parameters are met, intervention and notification:
Fever or hypothermia for the route used.
Tachycardia or bradycardia; a new or irregular rhythm; an apical–radial deficit.
Tachypnea or bradypnea; an irregular respiratory pattern.
Blood pressure outside the ordered parameters; symptomatic hypotension; an orthostatic drop.
SpO2 below the ordered target, or a drop of ≥ 4% from baseline.
Pain above the patient's goal.
A weight change meeting the at-risk threshold.
An adverse trend across visits — a rising respiratory rate, a climbing or falling blood pressure, or a steady weight gain — even when individual values look acceptable.
