Measure

• The full parameter set with technique noted; repeat any abnormal value with correct technique.
• Orthostatic blood pressure and heart rate when dizziness or falls are present.
• Apical pulse for a full minute with apical–radial deficit when the rhythm is irregular.
• Pre- and post-intervention values when an intervention is delivered (for example after a PRN antihypertensive or antipyretic).
• The cross-visit trend for the relevant parameter.
