4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Temperature 36.1–37.2 °C (97–99 °F) for the route used.
  • Heart rate 60–100 and regular.
  • Respiratory rate 12–20 and unlabored.
  • Blood pressure within the patient-specific ordered parameters (the general guideline target is below 130/80 mmHg, but the ordered parameters control).
  • SpO2 at or above 95% on room air, or at the ordered oxygen target.
  • Pain at or below the patient's goal.
  • Weight stable relative to baseline and target.
  • Vital-sign trend stable across recent visits.
