11. Physician or provider notification

Notify the provider per the patient-specific parameters and agency policy, and document the closed loop — contact, content, response, and follow-through.

  • State the specific parameter that was met (the value or finding that triggered notification).
  • Record the date and time of contact, who was notified, exactly what was reported, the response or orders received, and the follow-through.
  • If a response is pending, document the scheduled follow-up time so the loop is not left open.

Common notification triggers for this system: any value outside the ordered parameters; symptomatic hypotension or a positive orthostatic drop; SpO2 below the ordered target or a drop of ≥ 4%; a heart rate or respiratory rate beyond the ordered limits; a fever per parameters; a weight change meeting the at-risk threshold; or an adverse cross-visit trend even when individual values appear acceptable.

Closed-loop example

“Provider notified at 11:00 of a blood pressure of 178/98 (ordered parameter to report above 160 systolic), repeated with correct technique at 174/96, with the patient asymptomatic, alongside a three-visit upward trend. Order received to adjust the antihypertensive and recheck at the next visit. Patient educated on adherence and the report threshold; reassessment of blood pressure and the trend arranged.”
