Chest pain or pressure: onset, location, radiation, quality, severity 0–10, duration, provoking and relieving factors, and associated symptoms (diaphoresis, nausea, dyspnea).
Dyspnea: at rest versus exertion; the distance or activity that provokes it; orthopnea (number of pillows); PND; nocturnal cough.
Palpitations: frequency, duration, regularity, and associated lightheadedness or syncope.
Edema and weight: new or worsening swelling, the pattern, tighter rings or shoes, and any weight-gain pattern.
Dizziness, syncope, or near-falls with position change.
Activity-tolerance change; claudication (distance to symptom and relief with rest).
Adherence and adverse effects of diuretics, antihypertensives, beta-blockers, anticoagulants, and nitrates, and barriers (cost, dexterity, cognition, dosing complexity).
