Measure

  • Heart rate (rate and rhythm; apical for a full minute if irregular; apical–radial deficit).
  • Blood pressure (arm, position, technique); repeat if elevated; orthostatics if dizziness or falls.
  • Respiratory rate, SpO2 (room air and on oxygen if used), temperature, and pain score.
  • Daily weight versus prior and versus target/dry weight.
  • Heart sounds (S1/S2; S3/S4; murmur grade and location; rub).
  • Edema grade (1+ to 4+) and level; pulses graded (0–4+) bilaterally.
  • Functional tolerance: distance ambulated, stairs tolerated, or the activity at symptom onset.
