Cardiovascular conditions are among the most common reasons patients receive skilled home-health care, and cardiovascular decompensation is typically preceded by days of subtle, measurable change — a two- to three-pound overnight weight gain, a new S3, rising jugular venous pressure, an SpO2 that drops with exertion. Recognizing those signals before they become an emergency requires nursing judgment, not a layperson's observation.

The purpose of this module is to give clinicians the skilled framework and the chartable language that demonstrate cardiovascular assessment, monitoring, teaching, and coordination were reasonable and necessary and required the skills of a licensed nurse under the Medicare home-health benefit. It is the primary home for cardiovascular findings; related findings are cross-linked, not duplicated, into the peripheral vascular and disease-overlay modules.
