9. Patient and caregiver teaching

Document each teaching point with topic, method, audience, and measured mastery (teach-back percentage or return-demonstration result), per Foundations — Teach-back and return demonstration.

  • Daily weight monitoring: same scale, time, and clothing; logging; thresholds (> 2–3 lb overnight or > 5 lb per week) and whom to call.
  • Low-sodium diet: target, label reading, and hidden sodium; fluid-restriction adherence when ordered.
  • Medication regimen: purpose, timing, missed-dose instructions, not stopping beta-blockers abruptly, and adherence aids.
  • Home blood-pressure self-measurement with return demonstration: cuff placement, rest, position, logging, and provider thresholds.
  • Heart-failure zones (green / yellow / red) as a symptom-action plan.
  • Anticoagulation safety: bleeding precautions, interactions, dietary consistency (warfarin), the lab schedule, and when to seek care.
  • Activity pacing and energy conservation; gradual, symptom-limited progression.
  • Symptom recognition and emergency response: call 911 promptly for chest pain or other suspected ACS symptoms, and take prescribed nitroglycerin per order while awaiting EMS.
