Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.
Comprehensive hemodynamic and volume-status assessment with interpretation of the weight trend against baseline and target.
Application of parameter-based standing orders (for example, a PRN diuretic per weight or symptom protocol) with pre- and post-intervention monitoring.
Orthostatic vital-sign assessment and fall-risk mitigation when the patient is symptomatic.
Anti-anginal or nitrate administration per order with reassessment of pain and blood pressure.
Rhythm and rate assessment with parameter-based escalation.
Medication reconciliation: duplicates, omissions, BP-elevating OTC products (NSAIDs, decongestants), and drug–drug or drug–food interactions (for example, anticoagulant plus NSAID, or warfarin with variable vitamin-K intake).
Compression or offloading application or adjustment when ordered.
Coordination of laboratory monitoring per order (for example, electrolytes with diuretics, INR with warfarin).
Point-of-care INR testing and warfarin dose management per anticoagulation protocol where applicable.
