10. Response and reassessment

Document the patient’s response and the outcome in measurable terms — retention relieved with a measured residual, a post-operative site healing over a stated interval, correct return demonstration of catheter care — not “tolerated well.”

State the reassessment plan: the voiding, site, and scrotal parameters to recheck, the post-operative course to follow, and the threshold for escalation. When a surgical emergency or complication appears, document the revised plan and the closed-loop escalation.
