Every male genitourinary entry should make the skilled need explicit. The medical-necessity formula combines the skilled action, the RN-level reason it required a nurse, and the measurable effect on the patient or the plan.
The formula
skilled action + RN-level clinical reason + measurable effect on the patient’s condition or plan
Worked examples
• Skilled recognition of acute severe testicular pain with swelling was required to activate urgent surgical evaluation for testicular torsion within the narrow window in which the testis can be saved.
• Skilled assessment of an inability to void with suprapubic distension was required to relieve acute urinary retention by catheterization per order, measuring the residual and preventing bladder and renal injury (cross-reference Urinary/Renal).
• Skilled recognition of disproportionate perineal pain with crepitus and systemic toxicity was required to activate EMS for Fournier’s gangrene, a life-threatening necrotizing infection (cross-reference External Genitalia).
