“Normal/at-baseline” means voiding and post-operative recovery are on track and there is no acute scrotal, hernia, or infection finding. Each line states the finding, the comparison to baseline, and the skilled reason a nurse was required — a bare “no complaints” does not qualify.
Voiding at the patient’s baseline for known prostatic disease, without new retention or an inability to void.
No suprapubic distension; post-void residual within the expected range where measured (bladder detail in Urinary/Renal).
Scrotum and testes without acute pain, new mass, or significant new swelling; any chronic finding unchanged.
No new or incarcerated inguinal hernia; any reducible hernia unchanged from baseline.
A post-operative urologic site or catheter healing or functioning without bleeding, infection, or obstruction.
Genital and perineal skin intact without signs of infection (detail in External Genitalia).
Pain absent or chronic and stable, and the plan appropriate to continue.
