Subjective — ask

  • Dyspnea: at rest versus exertion; the distance or activity that provokes it; orthopnea (number of pillows); PND; nocturnal symptoms.
  • Cough and sputum: onset, color, amount, viscosity, hemoptysis, and pleuritic chest pain.
  • Device and adherence: inhaler, spacer, nebulizer, oxygen, CPAP or BiPAP use; barriers (cost, dexterity, cognition).
  • Triggers and exposures: smoking, allergens, cold air, occupational or home exposures.
  • Fatigue and activity tolerance, and any change from baseline.
