Escalation must follow the patient's orders and agency policy; the following are representative thresholds.
🚩 Red flags — escalate SpO2 below 90% on usual settings, or a drop of ≥ 4% from baseline with dyspnea — provider notification and possible same-day intervention. Respiratory rate above 28 or below 10, labored breathing, or inability to speak in full sentences — urgent evaluation. Acute dyspnea, pleuritic chest pain, cyanosis, or confusion — activate emergency response (911). Temperature above 100.5 °F with focal crackles or a change in sputum — provider notification for possible pneumonia. Hemoptysis — urgent evaluation; massive hemoptysis is an emergency. One-sided absent breath sounds with tracheal deviation — activate emergency response (911) for possible tension pneumothorax. |
