Objective — observe

• Work of breathing: accessory-muscle use, tripod posture, pursed-lip breathing, and full-sentence speech.
• Device use: oxygen delivery and cannula placement, spacer steps, nebulizer setup, and mask fit.
• Cyanosis, digital clubbing, and chest-expansion symmetry.
• Mental-status change suggesting hypoxia or hypercapnia.
