8. Interventions performed

Document the skilled interventions delivered this visit; each should connect to a finding and carry a reassessment in Section 10.

  • Oxygen titration within ordered parameters, recording the start-to-end flow and the reason.
  • Nebulizer or bronchodilator administration with pre- and post-intervention monitoring.
  • Correct inhaler and spacer technique with return demonstration.
  • Breathing retraining — pursed-lip, diaphragmatic, and paced breathing.
  • Airway-clearance coaching when ordered (huff cough, positive expiratory pressure).
  • Positioning (high-Fowler or tripod) for distress; energy conservation and activity pacing.
  • Medication reconciliation, including inhaler-device appropriateness and review of CNS depressants that may suppress respiration.
  • Coordination of laboratory or imaging studies per order (for example chest x-ray, sputum culture).
