10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• The effect of any coordinated intervention on the constitutional symptom and the gestalt.
• Tolerance and any adverse effects of a related medication.
• Teaching effectiveness: teach-back of the constitutional warning signs and the call plan.
• Functional response — any change in activity tolerance or assistance required.
• The plan for reassessment timing and what will be re-evaluated next visit.
