Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.
- The effect of any coordinated intervention on the constitutional symptom and the gestalt.
- Tolerance and any adverse effects of a related medication.
- Teaching effectiveness: teach-back of the constitutional warning signs and the call plan.
- Functional response — any change in activity tolerance or assistance required.
- The plan for reassessment timing and what will be re-evaluated next visit.
