Notify the provider per the patient-specific parameters and agency policy, and document the closed loop — contact, content, response, and follow-through.
State the specific parameter that was met (the value or finding that triggered notification).
Record the date and time of contact, who was notified, exactly what was reported, the response or orders received, and the follow-through.
If a response is pending, document the scheduled follow-up time so the loop is not left open.
Common notification triggers for this system: a fever per parameters, especially with tachycardia, tachypnea, or altered mentation; significant unintentional weight loss; a marked decline in function or self-care; new profound fatigue or an ill or toxic appearance; or any constitutional symptom cluster suggesting a new systemic process.

“Provider notified at 12:30 of a temperature of 101.4 °F with a heart rate of 108, a respiratory rate of 24, and new mild confusion — a cluster concerning for sepsis. Order received to evaluate for a source, obtain laboratory studies, and monitor closely, with instruction to activate emergency response for hemodynamic deterioration. The patient and family were educated on warning signs; reassessment arranged within 24 hours.”
