Normal findings The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.
Well-appearing and in no acute distress. Appropriately groomed with intact self-care. Alert and interactive, engaging appropriately. Apparently well-nourished and hydrated. Denies fever, chills, unintentional weight change, appetite loss, and night sweats. Functional status at the patient's baseline. Overall presentation stable and consistent with baseline.
