4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Well-appearing and in no acute distress.
  • Appropriately groomed with intact self-care.
  • Alert and interactive, engaging appropriately.
  • Apparently well-nourished and hydrated.
  • Denies fever, chills, unintentional weight change, appetite loss, and night sweats.
  • Functional status at the patient's baseline.
  • Overall presentation stable and consistent with baseline.
