The general survey is the clinician's first and broadest data point — appearance, distress, grooming, alertness, and the constitutional symptoms of fever, fatigue, unintentional weight change, and appetite loss. Their value is in the cluster: fever with tachycardia, tachypnea, and new confusion is a sepsis signal; new fatigue with weight loss and night sweats warrants evaluation. Recognizing when “something is off” and which cluster it forms requires nursing judgment and drives the focused assessment that follows.
The purpose of this module is to give clinicians the skilled framework and chartable language that demonstrate the general survey and constitutional review were reasonable and necessary and required the skills of a licensed nurse. It is the primary home for the overall clinical gestalt and constitutional symptoms; specific system findings are cross-linked, not duplicated, into the Body System modules, and weight and intake interpretation is shared with the Vital Signs and Nutrition domains.
