- Fluid intake — type and amount, and adherence to a restriction or an encouragement plan.
- Losses — vomiting, diarrhea, sweating, or high wound or ostomy output.
- Urinary changes — frequency, amount, color, or new symptoms.
- Adherence to and any difficulty with the ordered fluid plan; medication and heat or illness effects.
