Subjective — ask

• Thirst, dry mouth, dizziness, or lightheadedness; fatigue or weakness.
• Fluid intake — type and amount, and adherence to a restriction or an encouragement plan.
• Losses — vomiting, diarrhea, sweating, or high wound or ostomy output.
• Urinary changes — frequency, amount, color, or new symptoms.
• Adherence to and any difficulty with the ordered fluid plan; medication and heat or illness effects.
