10. Response and reassessment

Every intervention and teaching action requires a documented response that closes the loop. An intervention without a reassessment is an audit gap.

• The effect of an intervention — for example improved hydration signs after encouraged intake, or reduced overload signs after the volume plan.
• Tolerance of an intravenous-fluid or restriction change.
• Teaching effectiveness: teach-back of the fluid plan and the warning signs.
• Symptom response — resolution of dizziness, or improved urine output.
• The plan for reassessment timing and what will be re-evaluated next visit.
