11. Physician or provider notification

Notify the provider per the patient-specific parameters and agency policy, and document the closed loop — contact, content, response, and follow-through.

  • State the specific parameter that was met (the value or finding that triggered notification).
  • Record the date and time of contact, who was notified, exactly what was reported, the response or orders received, and the follow-through.
  • If a response is pending, document the scheduled follow-up time so the loop is not left open.

Common notification triggers for this system: signs of significant dehydration or an orthostatic drop; an inability to take or retain fluids; signs of fluid overload (new edema, weight gain, or dyspnea); high ongoing losses from vomiting, diarrhea, or drainage; or electrolyte-related symptoms.

Closed-loop example

“Provider notified at 11:30 of dry mucous membranes, a symptomatic orthostatic drop, and reduced, dark urine in a patient on a diuretic, with a 3-lb weight loss. Order received to hold the next diuretic dose, encourage oral intake, and obtain a basic metabolic panel. The patient was taught dehydration warning signs and intake strategies; reassessment and repeat orthostatics arranged.”
