Objective — observe

• Mucous membranes, skin turgor, and capillary refill.
• Edema, jugular venous distension, and lung sounds (examined in Cardiovascular and Respiratory).
• Urine color and concentration; the condition of any drainage source.
• Mental status and the ability to access fluids independently.
