4. Normal findings

The reference range below is the yardstick for “normal/at-baseline.” Normal findings are documented in Mode A with the skilled rationale that follows in Section 6 — never as “WNL” alone.

  • Moist mucous membranes, normal skin turgor for age, and no excessive thirst.
  • Intake adequate to meet needs, or within the ordered restriction.
  • Urine output adequate, with normal color and concentration.
  • Weight stable on the trend.
  • No signs of fluid overload (no new edema, jugular venous distension, or crackles) and no signs of deficit.
  • The ordered fluid plan tolerated and followed.
