Fluid balance sits at the center of many home-health diagnoses, and both dehydration and fluid overload develop quietly. An older adult with reduced thirst and a diuretic can slide into dehydration with dizziness and falls; a heart-failure patient who exceeds the fluid limit can retain volume toward decompensation. Distinguishing deficit from overload, and reading the trend, requires nursing judgment rather than a single observation.
The purpose of this module is to give clinicians the skilled framework and chartable language that demonstrate hydration assessment, intake-and-output interpretation, and fluid-plan management were reasonable and necessary and required the skills of a licensed nurse. It is the primary home for fluid-balance assessment; the volume-overload interpretation specific to heart failure is cross-linked, not duplicated, into Cardiovascular and the CHF overlay, and renal handling into Urinary / Renal.
