No outflow color, no clot/distension surveillance, no output correlation.

The post-TURP model note in §3: the outflow color and clearing, the patency and traction, the output correlation, and the reason skilled irrigation monitoring was required.

“Bloody urine.”

No color trend, no clot/distension check, no action, no anticoagulation note.

The hemorrhage model note in §4: the bright-red outflow with clots and distension, the hand-irrigation, the notification, and the anticoagulation review.

“Scrotum swollen.”

No post-op vs acute distinction, no emergency screen.

Distinguish expected post-operative swelling (supported, no hematoma) from an acute scrotal emergency requiring urgent evaluation.
