3. Normal findings — documented with skill justification

Each normal line pairs the finding with the reason a licensed clinician was required and why today, and states values against the ordered parameter or the specific prior value — never bare “WNL” or “at baseline.”

Model normal 1

Post-TURP with continuous bladder irrigation, day 1: the irrigation running per order with the outflow light pink and clearing (no gross clots), the catheter patent and secured with the traction/order maintained, output adequate and correlated with the irrigant, and no bladder distension or new pain. Licensed monitoring of the irrigation outflow and catheter patency and surveillance for hemorrhage/clot retention required — a covered daily skilled procedure. Model normal 2

Post-scrotal-surgery care: the surgical site clean and dry with expected mild swelling supported per order, no hematoma or excessive drainage, pain controlled, and scrotal support in place. Skilled post-operative site assessment and care required; supports Section I and the goal.
