Assess the male GU/genital status in the context of the skilled reason and document the judgment:
For post-prostatectomy/TURP with continuous bladder irrigation: the outflow color and clarity (bleeding/clots), the irrigation flow and catheter patency, and intake/output; for a catheter, the securement and site.
Signs of hemorrhage or clot retention (bright-red outflow, clots, bladder distension, no outflow, pain) and post-operative complication signs.
For scrotal/testicular surgery: the site, swelling, and pain; for the dependent resident, genital skin integrity (breakdown, moisture, catheter-related pressure).
MDS-supportive data
Catheter/continence is coded in MDS Section H, genital skin/wound in Section M, and the urologic diagnosis or recent surgery in Section I. The record must document the irrigation/catheter management, the hematuria surveillance, and any skin/wound so the MDS is supported.
