17. Common audit and denial risks

The patterns below are the system-specific failure modes; each maps to Segment 6 (Denial Patterns).

• Reaction status charted qualitatively (“no reaction”) without an action-plan or auto-injector check.
• An auto-injector charted as present without expiration or technique verification.
• A reaction or a fever in immunosuppression noted without action or the correct urgency (no closed loop).
• Repetitive, cloned allergic/immune notes with identical wording and no trajectory.
• Teaching documented as “educated on allergies” without learner response or return demonstration.
• A stable patient charted without an articulated ongoing skilled need (no readiness, surveillance, or teaching rationale).
