N4 Vaccine status current | Skilled assessment of vaccine status within the patient’s plan. The recommended vaccinations are current per the plan and there is no contraindication at this time. N5 Anaphylaxis action plan mastered | Skilled assessment of a patient newly prescribed an epinephrine auto-injector. The patient, after coaching, correctly described when and how to use the auto-injector and that EMS is called immediately after; the device is present and not expired. The mastery of the plan on return demonstration confirms readiness; the epinephrine-first rule, positioning, and follow-up were reinforced, with the patient and caregiver return-demonstrating correctly. |
N6 Allergen avoidance effective | Skilled assessment of a patient with a known allergen. The identified triggers have been removed or avoided in the home and there have been no reactions since the avoidance plan was implemented. The absence of reactions with effective avoidance, compared to baseline, confirms the plan is working; the avoidance measures and the action plan for an accidental exposure were reinforced, with the patient verbalizing them correctly. |
N7 Post-reaction recovery, monitoring | Skilled assessment following a prior allergic reaction that was treated. The patient has fully recovered with no residual symptoms; the trigger was identified and an avoidance and action plan established; the auto-injector was replaced and is not expired.
