  • Skilled dysphagia screening and aspiration-risk assessment with trajectory tracking against baseline.
  • Verification and correction of IDDSI-texture adherence and thickened-liquid preparation.
  • Skilled teaching of safe-swallow strategies, positioning, pacing, and caregiver feeding technique, to competence with return demonstration.
  • Immediate response to acute choking and recognition and escalation of aspiration and neurogenic dysphagia.
  • Coordination with speech-language pathology for re-evaluation and diet modification and with Nutrition for intake and weight.
  • Coordination with the provider for aspiration-pneumonia evaluation and for a swallow study when indicated.
