10. Response and reassessment

Document the patient’s response and the swallow outcome in measurable terms — tolerated the ordered texture without signs across the meal, caregiver return-demonstrated safe feeding, aspiration signs recognized and the diet held — not “tolerated well.”

State the reassessment plan: the swallow-screen parameters and texture adherence to recheck, the SLP re-evaluation status, and the threshold for escalation or diet change. When aspiration risk appears, document the revised plan and the closed-loop coordination.
