Dysphagia is a leading cause of aspiration pneumonia, hospitalization, and death in home-health patients, and it is frequently silent — a patient can aspirate without an obvious cough. The skilled value of this domain is screening for unsafe swallowing, verifying that the ordered texture and feeding precautions are protecting the patient, and teaching caregivers to feed safely. Documentation that records “eats fine” without a swallow screen does not prove a nurse was required; recognizing aspiration risk and protecting the airway does.
The purpose of this module is to give the clinician the framework for dysphagia screening, aspiration-risk assessment, IDDSI-adherence verification, and the recognition of choking and aspiration — and the chartable language that demonstrates observation and assessment and skilled teaching and training, in coordination with speech-language pathology, with neurogenic causes routed to Neurological and intake to Nutrition.
