- Coughing, choking, or a wet voice with or after eating, and any change since the last visit.
- Shortness of breath after meals, recurrent chest infections, or low-grade fevers.
- Adherence to the ordered texture, thickened liquids, positioning, and feeding precautions.
- Taste change or reduced appetite, and any sudden difficulty swallowing with other neurologic symptoms.
