Physician or provider notification Notify the provider and speech-language pathologist for a new unsafe-swallow sign, texture consumed beyond the ordered level, a decline in intake or weight from dysphagia, or signs of possible aspiration; document the finding, the time, the orders received, and the follow-through.
Escalate emergently and activate EMS for choking with airway obstruction, sudden dysphagia with neurologic signs, or acute aspiration with respiratory distress, and document the closed loop.
