Physician or provider notification Notify the provider and physical therapist for a new functional decline, a new fall, new weakness, uncontrolled pain, or a precaution that cannot be maintained safely; document the finding, the time, the orders received, and the follow-through.
Escalate emergently and activate EMS for a suspected fracture, compartment-syndrome or cauda-equina signs, or a suspected prosthetic dislocation, and document the closed loop.
