Musculoskeletal conditions drive a large share of home-health admissions — joint replacements, fractures, arthritis, and the deconditioning that follows illness or hospitalization — and the skilled need is often under-documented because a note records that the patient “ambulated” without proving the assessment and judgment a nurse brought to it. Strength graded against baseline, gait assessed for safety, precautions verified, and a functional decline recognized and acted on are evidence of skilled care; “walked in the hall” is not.
The purpose of this module is to give the clinician the skilled framework — inspection, palpation, range of motion, manual muscle testing, gait and balance assessment, assistive-device fit and use, and post-operative precaution verification — and the chartable language that demonstrates observation and assessment, skilled teaching, and management and evaluation of the plan of care, in coordination with therapy.
