Every musculoskeletal entry should make the skilled need explicit. The medical-necessity formula combines the skilled action, the RN-level reason it required a nurse, and the measurable effect on function or the plan.
The formula
skilled action + RN-level clinical reason + measurable effect on the patient’s condition or plan
Worked examples
• Skilled assessment of manual muscle strength was required to determine that right-hip-flexor strength improved from 3/5 to 4/5 post-operatively and that the patient could progress to the next activity level safely under the current order.
• Skilled gait and balance assessment was required to identify new instability and two near-falls, prompting a physical-therapy re-evaluation order and a temporary increase in transfer assistance that prevented an injurious fall.
• Skilled teaching of posterior-hip precautions and safe transfer technique, with return demonstration, was required to prevent prosthetic dislocation in the early post-operative period and to keep the recovery on its expected trajectory.
