13. Skilled-need justification

Observation and assessment

Grading strength, assessing range of motion, judging gait safety, verifying post-operative precautions, and recognizing a functional decline or a fracture, septic joint, or dislocation require the knowledge and judgment of a licensed nurse. Serial functional assessment against baseline is the core of observation and assessment for this domain.

Teaching and training

Teaching and reinforcing the home-exercise program, post-operative precautions, safe transfers, body mechanics, assistive-device use, and fall prevention — to patient and caregiver competence with return demonstration — is skilled teaching and training.

Management and evaluation of the plan of care

Evaluating whether mobility and strength are progressing as expected, and coordinating revisions to the activity order, therapy plan, pain regimen, or safety measures when they are not, is skilled management and evaluation of the plan of care.

Skilled procedures

Skilled procedures in this domain are limited but include splint or brace application and management per order, and post-operative incision care; most hands-on rehabilitation is delivered by therapy and reinforced by nursing.
