• Skilled hematologic assessment correlated with laboratory trends and trajectory tracking against baseline.
• Recognition and emergency escalation of significant bleeding, severe anemia, neutropenic fever, and thromboembolism.
• Home transfusion monitoring per order where applicable.
• Skilled teaching of bleeding precautions, infection precautions, and what to report, to competence with return demonstration.
• Coordination of anticoagulation with its overlay and immune compromise with Allergic/Immunologic.
• Coordination with the provider and oncology or hematology for laboratory-driven changes.
