Hematologic problems present quietly and turn dangerous quickly: occult bleeding produces fatigue and pallor before it produces a crisis, thrombocytopenia produces spontaneous bruising, and a single fever in a neutropenic patient is an emergency. The skilled value of this domain is surveillance — recognizing bleeding, anemia, and infection risk against the laboratory trend and acting before decompensation. Documentation that records “no bleeding” without the assessment and the anticoagulation or immune context does not prove a nurse was required; recognizing occult bleeding or a neutropenic fever does.
The purpose of this module is to give the clinician the framework for assessing anemia, bleeding, bruising, clotting tendency, and infection susceptibility — and the chartable language that demonstrates observation and assessment and teaching and training, with anticoagulation routed to its overlay and immune compromise to Allergic/Immunologic.
