Physician or provider notification Notify the provider for new or worsening anemia signs, minor bleeding, new bruising or petechiae, or laboratory trends indicating falling counts; document the finding, the time, the orders received, and the follow-through.
Escalate emergently and activate EMS for active significant bleeding, severe-anemia symptoms, a neutropenic fever, a transfusion reaction, or suspected thromboembolism, and document the closed loop.
