Hematologic conditions support homebound status through anemia-related exertional limits and infection precautions; document the patient-specific evidence when present and do not overstate it.

Hematologic-related evidence that supports homebound status

  • Severe anemia with fatigue and exertional dyspnea that limit the ability to leave home and require a taxing effort.
  • Immunosuppression requiring infection precautions that restrict leaving home.
  • A bleeding disorder or high bleeding risk requiring skilled monitoring that limits safe travel.
  • A need for assistance and a taxing effort to leave home where hematologic management is ongoing.
