  • Distinguishing incontinence-associated dermatitis from a pressure injury and applying the correct barrier or offloading treatment.
  • Perineal wound care and device entry-site care per order (staging in Wounds; catheter care in Urinary/Renal).
  • Skilled teaching of perineal skin care, barrier application, moisture management, and the warning signs to report, to competence with return demonstration.
  • Coordination with the provider and wound care and cross-reference to Wounds, Urinary/Renal, and Integumentary.
