10. Response and reassessment

Document the patient’s response and the skin outcome in measurable terms — dermatitis improving over a stated interval with the barrier, a wound healing, correct return demonstration of perineal care — not “tolerated well.”

State the reassessment plan: the skin, wound, and device-site parameters to recheck, the barrier plan to confirm, and the threshold for escalation. When a necrotizing infection, a deepening wound, or worsening dermatitis appears, document the revised plan and the closed-loop escalation.
