“Normal/at-baseline” means the perineal and genital skin is intact or a known lesion is healing, the barrier plan is effective, and there is no infection. Each line states the finding, the comparison to baseline, and the skilled reason a nurse was required — a bare “skin okay” does not qualify.

No incontinence-associated dermatitis, or a known dermatitis improving with the barrier protocol.
Any perineal or genital wound healing on the expected trajectory (staging in Wounds).
The catheter or device entry-site skin intact without erythema, breakdown, or trauma.
No signs of a necrotizing or fungal infection.
Moisture controlled with an effective barrier and containment plan.
Hygiene maintained, the at-risk skin protected, and the plan appropriate to continue.
